Drayton Medical PracticeWe are currently running a campaign using Mjog (our text message / smartphone app tool) to ask patients if they would like an Enhanced Summary Care Record (eSCR).
Most patients already have a basic Summary Care Record that allows clinicians in other health care settings to view information about:
- recent medication
- Allergies
- Sensitivities to medications
By enhancing the Summary Care Record extra information can be shared such as:
- Long Term Health Conditions
- Relevant Medical History
- Healthcare Needs and Personal Preferences
- Immunisations
If you have received a text or message from us about this please follow the instructions in the message if you want to opt in. Alternatively you can complete our online form.
If you don’t want to enhance your Summary Care Record you can ignore the message.
If you don’t receive a message but would like an Enhanced Summary Care record please use our online form or let us next time you call or attend the surgery.
