New Patient Registration

If you would like to register with our practice please fill out the relevant form via the links below.

To register a new patient you will need to live within our practice boundary:

You will also need to upload a photo of your photo identification (driving license or passport) or if this is not available a birth and or marriage certificate and proof of address (utility bill, bank letter or similar) when completing your registration form. Alternatively, you can hand these in at the practice.

Please note that if we do not have your NHS number this may delay registering you (and your family) at the Practice.

Upon registration you will be assigned an accountable GP. This will be Dr Pearson although you may of course, request to see any other clinician of your choice.

Alternatively you may collect registration forms from any of our surgeries, once completed please return along with photo ID and proof of address. If you have problems with this, please do not hesitate to speak to a receptionist on 01603 867532.

NHS Health Check

For those patients aged between 40 and 74 years with no pre-existing conditions and who have not had a ‘NHS Health check’ in the last 5 years, please book a NHS health check on line or call our reception team.
